‘I’ referred to as the recipient, one purchasing the event ticket.

I understand that this is a consent form for accessing the event. The event will host various forms of energy healing deemed fit by Carly Carreiro henceforth referred to as ‘Spiritual Mentor’. The energy healing modalities include, Talk Therapy, Channeled Messages, Chakra Balancing, Ahai, Sound Healing and Light Language.

I understand the following: The energy healing (especially sound therapy) can have effects on certain medical conditions such as diabetes, pregnancy and/or having metal in the body. Energy healing can affect insulin levels, and affect screw, pins and plates in the body. I understand I should consult my medical provider on any medical conditions.

I hereby give my consent to participate in the event and understand that the services provided by my chosen Spiritual Mentor are intended to provide general relaxation

I understand completely that the services provided during the event are in no way a substitute for traditional medical treatment or advice. I am fully aware that my chosen Spiritual Mentor will not offer any diagnosis or recommend any Medical Treatment or Prescribed Medication. 

I understand that I must continue to have regular medical check-ups as part of my overall personal health care plan; and I should contact my own certified and licensed medical physician/doctor/health care professional for any physical or psychological ailments or concerns that I may have in order to get proper medical advice. 

I agree and understand that my participation in the event is voluntary and that at any time during the event I can choose to end my participation. I also understand that I may experience 'self-healing reactions' during the 48 hours following the event.

I understand that any information exchanged during any session is educational in nature and is to be used at my own discretion, information shared in the event is not medical, legal or financial advice.

Finally, I understand that by providing this informed consent I am assuming full responsibility for participating in the event and I hold harmless both my chosen Spiritual Mentor and Carly Spirit, the facility/location where the services are provided.
